Below is our application to request services from Latevia Nichols. Latevia works online, so applicants can be at any location in the United States.

- If you have Adobe PDF: download this document, go to “Tools”, click “Edit PDF”, complete the application, save it to your computer, and email to TheLegacyFitMom@gmail.com
- Because these are online services , you can also download either document, print it, complete the application by hand, scan it back into your computer, and then email it to Latevia.
